Grant Request
LAPS FOR LIFE, JAG, CHARITABLE ORGANIZATION GRANTS AND GIFTS
Laps For Life, JAG, is a charitable organization founded and incorporated in 2005. Laps For Life, JAG (referred to as LFL), is a non-profit corporation that has been granted 501C-3 Tax Exempt Status by the US Internal Revenue Service.
The LFL mission is to promote lifelong fitness while providing support to cancer patients and their families.
To support our mission, we grant monies and provide gifts to organizations who directly support cancer patients during their treatment and individuals who are undergoing treatment for the disease.
LFL GUIDELINES FOR FUNDING GRANTS TO INDIVIDUALS:
- Grants are made to individuals who are currently undergoing treatment for cancer or have completed a course of treatment within 12 months
- In order to be eligible for an individual grant, the requestor
must receive a recommendation by one of the following:
- Physician currently treating the patient
- Counselor or professional working with the patient and/or the patient’s family member(s)
- Support group or medical organization participating in patient’s care
- The maximum amount of an individual grant is $ 2,500
- Individuals may receive only one grant within a 12-month period
LFL GUIDELINES FOR FUNDING GRANTS TO ORGANIZATIONS:
- Grants will be made to charitable and non-profit organizations only
- Grant requests must demonstrate specific and direct benefits to patients undergoing treatment for cancer and/or support for their families
- Grants for research will not be accepted
- The maximum amount of an organizational grant is $2,500
- Organizations may receive only one grant within a 12-month period
Proposals for grants must be submitted in writing, on a LFL Request for Grant form (see below). Grant forms may be mailed to Laps for Life; 4356 Tuller Rd., Dublin 43017, or delivered to a LFL Board member.
